Incident in radiotherapy in Czechia: Connected to use of AI or conturing. Radiation oncologist wanted to add a new volume for conturing of target volume and instead of creating a new volume he used an AI tool for automated conturing and choose one of possible volumes in this tool. He intentionally used a volume of the eye lens as he knew that eye is very far from the target volume (parotides) and he won’t need this pre-defined volume for the planning. He used this pre-defined volume, changed its name and drew completely new volume inside it – to conture the parotides. But as he was in the slices around the parotides he forgot that this tool works that way that it draws the original volume despite the fact that it is far from the currently viewed slices.. So the mistake was not caused by the AI tool but by its wrong use. As this volume was far form the target volume, nobody noticed it later, so finally the eye lens was irradiated as a target volume with the almost full dose – resulting in need of its replacement and damage of tears-producing function of the eye. Lessons learned – being aware of the full scope of the functions of AI tools, and routine use of tools identifying far volumes.
